Stichting Met je hart wil eenzaamheid tegengaan door het mogelijk maken van waardevolle ontmoetingen voor kwetsbare ouderen. Met je hart zet zich in voor kwetsbare ouderen die zelfstandig wonen, moeilijk de deur uitgaan en weinig mensen ontmoeten. De kwetsbaarheid wordt versterkt door bijvoorbeeld een fysieke beperking, een klein sociaal netwerk, de financiële situatie of verlegenheid en angst.
