About HC@Home

HC@Home enables patients with chronic diseases to actively participate in their treatment using a easy-to-use solution for use in the home situation. Working on their health in their own environment, with remote supervision by healthcare professionals, gives the patient full flexibility to select activities that fit the personal circumstances and preferences. With HC@Home activity and vital signs such as blood pressure, heart rate and weight can be measured. The data are automatically transmitted to a secure website via a specially adapted smartphone. The measurements can be accessed at any time by both the patient and his healthcare professional. HC@Home maakt het mogelijk patiënten met chronische ziekten zelf de regie te geven in een deel van hun behandeling met behulp van een gebruiksvriendelijke oplossing voor toepassing in de thuissituatie. Thuis en in de eigen omgeving werken aan de gezondheid, met supervisie op afstand, geeft de patiënt volledige flexibiliteit activiteiten te ontplooien die aansluiten bij de persoonlijke situatie en voorkeuren. Met HC@Home kunnen thuis activiteiten, hartslag, bloeddruk en gewicht gemeten worden. De gegevens hiervan worden via een smartphone automatisch naar een beveiligde website gestuurd. Op ieder gewenst moment kunnen de metingen worden ingezien door zowel de patiënt als diens arts.
