About CIS - Centrum Indicatie Stelling
Centrum Indicatie Stelling stelt indicaties ZVW (thuiszorg) voor iedereen met een vraag voor persoonlijke zorg. Ook voor het aanvragen van een indicatie voor langdurige zorg (WLZ) staan wij voor u klaar.
Omdat we eigen regie en zelfredzaamheid van de cliënt bij ons hoog in het vaandel hebben staan, benaderen wij de cliënt vanuit zijn of haar eigen mogelijkheden en sociale netwerk. Daarom voert onze indicerend verpleegkundige altijd het gesprek “achter de voordeur”.
