WHY “When my doctor was concerned that my cancer had returned, 25 years after my last radiation as a child, I was wondering, does my current doctor know all my medical history like the doctor of my childhood knew? That is when my dream was born: “Any patient in the world should be able to give access to his medical data to any doctor in the world, so he can be assured of best medical help.”

HOW Lemon connects different EHR’s via blockchain technology. This interoperability brings an end to information silos with medical data. The data is still stored in the silos, only the reference and the authorisation are stored on the blockchain. This is done based on the information standard Registratie aan de Bron.

WHAT An app enables the healthcare professional to request access to the data of a patient. The patient can grant access after he received the request on his own device. Then the authorisation for this professional is stored on the blockchain. The patient has already indicated which health organisations are filing his medical data from the past. When the professional opens the Lemon app, he will see one timeline with all combined information of the health organisations where his multi-institutional medical data are stored now.

HOSPITAL @ HOME The h@h case reflects very much the wider case of a patient sharing data with several professionals. H@h requires the data sharing happens instant, as several health organisations are working together on the same treatment. That is why we are delighted that UMCG is very interested in continuing with us to enable h@h by data sharing via Lemon.

BLOCKCHAIN We choose for a blockchain solution because it is the only way to solve this issue.

Trust, security and immutability: enables the patient to decide who is granted which authorization. Once the authorization is stored in the blockchain it can not be changed anymore. This really empowers the patient!
